The name of the village is eponymously derived from the folklore word of Turkic origin "saltan", aka "sultan" in English. Most likely there was a proto-city here founded in the middle ages under the Khazar Kaghanate.

Until 18 July 2020, Staryi Saltiv belonged to Vovchansk Raion. The raion was abolished in July 2020 as part of the administrative reform of Ukraine, which reduced the number of raions of Kharkiv Oblast to seven. The area of Vovchansk Raion was merged into Chuhuiv Raion.[3][4]

In the 2022 Russian invasion of Ukraine, Saltiv was occupied by Russian forces early in the invasion, and became the scene of renewed fighting in the Battle of Kharkiv (2022). In 2 May 2022, an American defense official corroborated Ukrainian defense officials that Staryi Saltiv was retaken by the Ukrainian Armed Forces.[5]
